    Objective Carbapenem-resistant Enterobacteriaceae infections is becoming a major challenge to clinicians,which resulted in extremely high mortality owing to the limited antibiotics option.It is urgently to explore strategies to manage such bacterial infections.So far,the research is mainly focused on the research and development of new drugs,as well as the re-evaluation of classic antimicrobial drugs and the combined treatment regimens.Tigecycline is available in our country,but the clinical effect is still doubtful.Polymyxin has a good effect on drug sensitivity test in vitro,but the clinical dosing has not been established.Carbapenems combining with other drugs as optimizing treatment program is also being explored.Carbapenemase- inhibitor combination has not yet listed on Chinese market.In the mean time,it is urgent to prevent and to control carbapenem-resistant Enterobacteriaceae bacterial infection.

    Objective To provide reference for the adjustment of provincial health insurance directory by comparing the differences between 2017 and 2009 edition of National Health Insurance Directory(NHID) and exploring the characteristics of NHID adjustment. Methods The Excel 2016 was used to carry out a comparative analysis of the drugs included in the two editions of NHID from the 4 aspects such as classification,drug name,dosage form and payment range. Results The adjustments of NHID in 2017 include drug variety,payment range,drug classification and coding.A total of 339 drugs were added,of which 133 were chemical drugs with a growth rate of 11.4%,and 206 were Traditional Chinese Medicine with a growth rate of 20.0%.In addition,dosage form was added in 149 drug varieties.The adjustment mainly focuses on chronic diseases such as hypertension and diabetes,serious diseases such as cancer,innovative drugs,and pediatric drugs. Conclusion The adjustment of 2017 edition of NHID embodied the policy thinking of "filling,selecting,supporting innovation and encouraging competition".Moreover,the drug classification,name and coding are more scientific and reasonable.

    Objective Two co-crystals of isonicotinic acid hydrazide(INH)-malonic acid and INH-glutaric acid were prepared.The formation mechanism and intermolecular interaction were studied. Methods Three-dimensional structure of 2 co-crystals were obtained though single crystal X-ray diffraction(SXRD), and intermolecular interaction was analyzed using Hirshfeld surface method. Results INH-malonic acid crystalized in stoichiometric ratio of 1:0.5,while INH-glutaric acid in 1:1.Two co-crystals maintain their stable arrangement in space by hydrogen bond and van der Waals force. Conclusion With the existence of pyridine ring and carbohydrazide group in INH,which mainter-molecular interaction in co-crystal can be directly and clearly revealed by Hirshfeld surface analysis.

    Objective To observe the effects of total glucosides of white paeony(GWP) on psychological ethology and neuroendocrine system of stress rats. Methods Ten male Sprague-Dawley rats were treated as normal control group, the others(n=40) were constructed by isolated custody and chronic unpredictable stimuli to build the model of stress rats.Stress rats were randomly divided into 4 groups:model control group,GWP high dose group and low dose(66,33 mg·kg-1) group,fluoxetine(5.0 mg·kg-1) group,each group had 10 rats.The changes of rat behavior were monitored by locomotor activity system.The change of emotion was observed by tail suspension test.The pathology change of adrenal was observed by HE staining.The levels of CorT and ACTH in serum were measured by radioimmunoassay.The expression of CRH mRNA of hypothalamic were detected by RT-PCR. Results The activity of the level scoring and vertical scoring of GWP high dose group was obviously improved as compared to that in model control group,while static time was significantly shortened,Adrenal index of GWP groups were increased significantly(P<0.05) ,thymus index of the treatment group was declined(P<0.05),CorT and ACTH content were decreased (P<0.05) and hypothalamus CRH mRNA of GWP group was decreased(P<0.05) . Conclusion The improvement of activity and related hormone anomalous changes were observed in rats treated with GWP.The mechanism of its actions might be involved adjusting the endocrine function,decreasing CorT and ACTH content and the expression of hypothalamic CRH mRNA of stress rats.

    Objective To investigate the molecular mechanism of luteolin-regulated epithelial-mesenchymal transition(EMT) in non-small cell lung cancer. Methods Firstly,the potential target of luteolin was studied by molecular docking.The effect of luteolin on EMT markers was analyzed,and then molecular biology experiments were used to verify the results. Results The result of molecular docking showed that luteolin had a good docking effect on the integrin family,of which the lowest binding energy was -15.11 in docking with Integrin αIIbβ3.The result of flow cytometry showed that luteolin could down-regulate the expression of integrin induced by hypoxia. Conclusion Luteolin can regulate integrin expression ,which suggested that luteolin can regulate EMT through integrin.This conclusion may provide new methods in prevention of tumor metastasis for traditional Chinese medicine.

    Objective To discuss the effect of α-asarone on the expression level of Cyt-c,Smac,Caspase3 mRNA and protein in human esophageal carcinoma Eca-109 cell mitochondria. Methods The Eca-109 cells were cultured in vitro,and divided into the negative control group and the α-asarone treatment groups(final concentration:25,50,100 μg·mL-1).After 48 h,the morphological changes of Eca-109 cells were observed by fluorescence inversion microscope.The total RNA of cells were extracted by TRIzol method,the expressions of Cyt-c、Smac and Caspase3 were measured by RT-PCR and Western blotting. Results After Eca-109 cells were treated with different concentrations of α-asarone for 48 h,and obvious changes in the morphology were observed,the expressions of Cyt-c,Smac and Caspase3 genes and protein were increased significantly compared to the negative control group( P<0.05). Conclusion α-asarone can induce the human Eca-109 cells apoptosis by regulating expressions of mitochondrial apoptosis pathway correlation genes such as Cyt-c,Smac and Caspase3.

    Objective To investigate the effect of venetoclax on RNA expression and cytokines release in lymphocytes isolated from acute myeloid leukemia(AML) patients. Methods MTT method was performed to study the apoptosis of AML cells and optimize the concentration of venetoclax.The gene markers expression and cytokines production before and after venetoclax treatment were examined by RT-PCR and ELISA methods,respectively. Results After chemotherapy,the expression of tumor associated inhibitory genes including BCL-2(P<0.05),SIRPα(P<0.05),CD47(P<0.05),PDL-1(P<0.05),PDL-2(P<0.01),and EZH2(P<0.05) were significantly decreased when venetoclax reached 300 nmol·L-1.In addition,the expression levels of the activated molecules significantly increased,such as TRAIL(P<0.01) and TNF-α(P<0.01).The ELISA results showed that the release of human TNF-α and IFN-γ have significantly increased. Conclusion Venetoclax,which is a chemotherapeutic drug,can be used as tumor targeting drug.It can promote apoptosis in AML patients by regulating the genes expressions related to malignancy and cytokines production.

    Objective To research sorption of local anesthetic by fat emulsion with different content of lipid in vitro and then to discuss the mechanism of action of fat emulsion for treatment of local anesthetic toxicity. Methods 10%,20%,30% fat emulsion was added to the bupivacaine hydrochloride and ropivacaine hydrochloride.After the mixture was vortexed for 10 mins,it was vibrated in thermostatic water bath at 37 ℃ for 15 h ,then centrifugated and got the aqueous phase to HPLC test.The chromatography was carried on a C18 column(4.6 mm×250 mm,5 μm ),the mobile phase was composed of acetonitrile-natrium biphosphoricum(25:75),the flow rate was 1 mL·min-1,the column temperature was 30 ℃ and the detection wavelength was set at 210 nm. Results Under the condition of this test,the linear ranges and linear equations for bupivacaine hydrochloride and ropivacaine hydrochloride were 0.3-3.0 μg·mL-1,Y=0.093 6X-0.017 3(r=0.999 6) ,and 0.3-3.0 μg·mL-1,Y=0.086 6X-0.022 3(r=0.999 1) respectively.The lowest limit of quantitation were both 0.05 μg·mL-1(S/N>3),and the ingredients showed good relationships between the peak area and the concentration.Sample reproducibility was good because the test sample was stable in 5 h.The extraction rates of bupivacaine hydrochloride by 10%,20%,30% fat emulsion were 46.0%,70.4%,89.2% respectively.The extraction rates of ropivacaine hydrochloride by 10%,20%,30% fat emulsion were 51.3%,71.6%,90.7% respectively. Conclusion Electric potential and the pH value of the 10%,20%,30% fat emulsion are similar,while the particle size increase slightly.Free bupivacaine hydrochloride and ropivacaine hydrochloride can be absorbed and with the increase of fat content,the extraction of local anesthetic is increased.The mechanism of action of fat for the treatment of local anesthetic toxicity maybe related to that fat emulsions can provided lipid,so that the excessive local anesthetic or those which had already distributesd in tissue can re-dispersed in lipid,and then the plasma concentration of local anesthetic is reduced,the toxicity is also reduced.

    Objective To isolate and identificate Streptomyce strain from cow dung and observe its antimicrobial activity. Methods Strains were isolated from cow dung by dilution coating method.Strong antibacterial strains were screened out by agar block method with fixed Staphylococcus aureus and Escherichia coli as indicative bacteria.The strains were identified based on physiological and biochemical characteristics as well as 16S rDNA gene sequence analysis.Active antibacterial fermentation broth substance was determined by disk diffusion method,and antibacterial active substance of strains fermentation broth was extracted by water-saturated n-butanol.Antibacterial substance of strains was identified by Molish reaction,biuret test and ninhydrin reaction. Results Eight strains were isolated from cow dung and one strong antibacterial strain was screened out and named B5-2,identified as Streptomyces.The results showed that the strain had the highly antibacterial effect on Staphylococcus aureus,Escherichia coli,Citrobacter freundii,Enterobacter cloacae,Klebsiella pneumoniae,Enterobacter aerogenes.The strain antibacterial active substance of fermentation broth preliminary analysis showed that strong antibacterial active substance of B5-2 was the water-soluble substance.Antibacterial substance of B5-2 was preliminarily identified as glycoside and protein by Molish reaction,biuret test and ninhydrin reaction. Conclusion The strain isolated have a strong inhibition effect on clinical pathogenic bacteria in clinical practice.

    Objective To understand the current research of vancomycin against methicillin-resistant Staphylococcus aureus(MRSA) infection and to reflect major nations’ scientific advances and influence in the field. Methods The literatures were searched from Web of Science citation database,using the term of “vancomycin” and “methicillin-resistant staphylococcus aureus” (1997—2016).The results were analyzed by bibliometric method concerning the numbers of literatures and citations. Results A total of 5 049 literatures were recorded in Web of Science.The total number of publications and citations was increased quickly from 1997 to 2016.The major type was journal article,about 84.492%.The most studies were on the aspects of microbiology and pharmacology.USA had the largest numbers of publications,accounting for 42.523% of all related publications.Most institutions were from USA,which had the highest number of publications with the highest quality.China ranked the seventh by the numbers of publications. Conclusion USA plays a leading role in this field and the research in China still lags behind its international peers.

    Objective To provide reference for drug use in infant patients with Rhodococcus equi(R.equi) infection. Methods By participating in drug therapy for an infant patient with Rhodococcus equi infection,clinical pharmacist assisted physicians to develop treatment plan and provide reasonable pharmaceutical care. Results Therapeutic selection was improved through pharmaceutical care process. Conclusion Participation of clinical pharmacist can fully play their advantages in the field of drug efficacy,and improve the level of clinical treatment.

    Objective To observe the clinical effect of ropivacaine erector spinae plane block assisting general anesthesia in lumbar spine surgery. Methods A total of 60 cases of male patients with selective lumbar spine surgery under general anesthesia were chosen and randomly divided into control group(group C) and erector spinae plane block group(group E)(n= 30).Group C was directly treated with total intravenous anesthesia.In group E,the erector spinae were blocked with 0.375% ropivacaine before skin incision.The patient's vital signs during operative period were recorded,as well as the operation time,anesthesia time,drug dosage and satisfaction of muscle relaxation .Postoperative VAS scores at 4,8,24 and 48 h were recorded.Remedied analgesic drug use,the number of PCIA press,complications such as nausea and vomiting,patients’ satisfaction degree within 48 h of postoperative were all recorded. Results There were no statistical difference in the vital signs,operation section,operation time,anesthesia time and postoperative complications(all P>0.05) in two groups. Compared with group C,the dosage of propofol,rifentanyl and benzene sulfonate in group E were significantly reduced .In group C and group E,dosage of propofol was(1 526.2±134.5) mg and(1 305.8±212.8) mg; dosage of remifentanil was(2 452.2±232.5) μg and(2 076.8±311.6) μg; dosage of benzene sulfonate was(31.8±4.2) mg and (22.3±5.1) mg]( P<0.05 or P<0.01) .Satisfaction of muscle relaxation in the operation area of group E was significantly increased(P<0.05).The VAS scores at 4 h and 8 h after operation in group E were all significantly reduced(P<0.01).In group E,the frequency(or total) of remediation analgesic drugs usage and PCIA compressions times were significantly reduced in 48 h after surgery(P<0.01).The satisfaction degree of the patients in group E was significantly increased(P<0.01). Conclusion Ropivacaine erector spinae plane block assisting general anesthesia in lumbar spine surgery can reduce perioperative general anesthesia drug dosage,the number of postoperative remediation of analgesic drugs,PCIA compressions times,and can improve satisfaction of operative doctors and patients with anesthesia.

    Objective To evaluate the effects and safety of dezocine and dexmedetomidine hydrochloride on the prevention of the pediatric postoperative agitation. Methods A total of 90 pediatric patients undergoing prepuce cerclage were randomly divided into 3 groups(n=30):dezocine group( Group Dez ),dexmedetomidine group( Group Dex ) and control group( Group C ),all groups were implemented general anesthesia combining with penile dorsal nerve block anesthesia.After induction of anesthesia,Group Dez and Group Dex were given 0.1 mg·kg-1 of dezocine and 0.5 μg·kg-1 of dexmedetomidine hydrochloride,respectively,Group C was given 0.9% sodium chloride solution.The rate of pediatric agitation, the operating room,the recovery time,the amount of additional propofol during the operation and the adverse reaction incidence within 6 hours after the surgery(circulation and respiratory depression,drowsiness,headache,nausea and vomiting) were observed and recorded . Results All groups have the surgery successfully done.There were no significantly difference among the three groups on the recovery time( P>0.05) .The incidence of postoperative agitation was 3.33% in Group Dez,0.00% in Group Dex,46.67% in Group C ,respectively( P<0.05 ) .All of the pediatrics in three groups were not given additional propofol.There was no obvious adverse reaction at the time of 6 hours after surgery. Conclusion Dezocine and dexmedetomidine hydrochloride both can reduce the rate of postoperative agitation in pediatric patients and have no obvious side effects.Therefore,the clinical use of dezocine and dexmedetomidine hydrochloride are safe and effective.

    Objective To explore the efficacy of alprostadil combined with Bailing capsule in the treatment of early chronic kidney disease. Methods A total of 94 early stage chronic kidney disease patients were selected and divided into treatment group(n=46) and control group(n=48).The patients in control group were treated with Bailing capsule,5 capsules,tid,po.The patients in treatment group were treated with Bailing capsule combined with 2 mL alprostadil in 20 mL 0.9% sodium chloride injection,intravenous injection,qd.The patients were treated for 4 weeks as a course of treatment in both groups.After 2 courses of treatment,the improvement of renal function,the changes in cytokine levels including NK cells and T cell subsets C C$D^{+}_{3}$,C$D^{+}_{4}$,C$D^{+}_{8}$,adverse reactions of two groups were observed. Results The effective rates of the control group and the treatment group were 60.42%,91.30%,respectively(P<0.05).The renal function index 24 h urine protein were(1.15±0.35) g,serum creatinine were(78.52±10.63) μmol·L-1,urea nitrogen were(8.23±1.65) mmol·L-1,all of which were decreased significantly(P<0.05).The levels of NK cells were(21.89±2.73)%,T cell subsets C$D^{+}_{3}$ were(71.02±5.61)%,C$D^{+}_{4}$ were(38.84±3.52)%,C$D^{+}_{4}$/C$D^{+}_{8}$ were(1.28 ± 0.14),which were increased significantly,while the level of C$D^{+}_{8}$ were(30.21±3.03)% was decreased significantly(P<0.05).There was no significant difference between two groups in the adverse reactions(P>0.05). Conclusion The combination of alprostadil and Bailing capsule is effective to early stage chronic kidney disease by improving the renal function and regulating the level of cytokines.

    Objective To optimize the supercritical CO2 extraction technology condition of radix cynanchi paniculati. Methods The paeonol extract was used as indexes.The content of paeonol was determined by high performance liquid chromatography method.The extraction condition was optimize tby design-response surface method. Results The optimum process conditions for the extraction pressure was 10.81 MPa,extraction temperature was 55 ℃,flow rate of CO2 was 59.91 L·h-1,extraction time was 2.42 h. Conclusion The content of paeonol with optimum extraction process of radix cynanchi paniculati is high,process is stable and feasible.

    Objective To determine the solubility of rhein in different vehicles and its partition coefficients in the n-octanol-water system for designing new formulations. Methods High performance liquid chromatography(HPLC) method was established to determine the concentration of rhein in water,different pH solutions and different solvents;the partition coefficients for the n-octanol- buffer solution systems were determined by shaking flask method. Results The equilibrium solubility of Rhein was 3.89 μg·mL-1 and the lgP was 2.79 in water at 37 ℃.The solubility of rhein was increased with the raise of pH ,which could reach 362.20 and 431.65 μg·mL-1 in phosphate buffer solution at pH 6.8 and 7.4,respectively.Solubility of rhein in the semi polar solvent was relatively good,which can up to 2 971.74 μg·mL-1 in PEG400.The oil-water partition coefficients of rhein were decreased with the increase of pH,and were 0.83 and 0.54 in phosphate buffer solution at pH 6.8 and 7.4,respectively. Conclusion Rhein is almost not soluble in water.With the raise of pH,the solubility is increased but the oil-water partition coefficient is decreased.

    Objective To develop a method for determination of gallic acid and ellagic acid in three kinds of Yitirifeili preparation. Methods HPLC method was used to determine the content of gallic acid and ellagic acid.The analysis was performed on Waters XBridge ShieldRP C18 column(4.6 mm×250 mm,5 μm) with the mobile phase consisted of methanol- acetonitrile-0.2% phosphoric acid in gradient elution manner.The flow rate was 1.0 mL·min-1,the column temperature was 25 ℃,and the detection wavelength was 254 nm. Results The linear ranges of gallic acid and ellagic acid were 0.132-0.792 μg,0.108-0.648 μg,respectively.Both correlation coefficient were more than 0.999 9.The average recovery of gallic acid and ellagic acid were 98.11%(n=9),RSD=1.39%; 99.32%(n=9),RSD=2.48%. Conclusion The method is accurate and reliable.It can be used as the quality control of Yitirifeili preparations.

    Objective To explore the effects of smoking on medications and provide an accurate and reliable evidence for smoking patients. Methods The drug instructions were surveyed from Datong medical drug counselling software database,and then the effects of smoking on drugs were analyzed by the drug instructions combined with the literature reports. Results There were 48 drugs varieties which might interact with smoking in Chinese drug instructions filtered from Datong medical drug counselling software database,which was much less than the report surveyed based on the FDA drug instructions(188 drugs varieties),there were another 37 varieties that off the instructions,reported by literatures interacted with smoking.Analysis revealed that smoking effects on drugs mainly by means of changing the pharmacokinetics and reducing the efficacy through inducing the CYP1A2,increasing the morbidity of some diseases such as cardiovascular disease,and increasing the adverse drug reactions.It was found that dosages of 8 drugs varieties need to be adjusted in smokers which were mainly or partly metabolized by CYP1A2.They are hydrochloride erlotinib,theophylline,riociguat,insulin,warfarin,clozapine,olanzapine and chlorpromazine.Dosage of 9 drugs varieties may need to be increased in smokers. Conclusion For the purpose of rational drug use,dosage of a variety of drugs should be adjusted in smokers.

    Objective To investigate the unusual drug-induced International Normalized Ratio(INR) change in elderly patients with warfarin treatment and its related mechanism. Methods A retrospective analysis was performed in 41 cases of elderly patients with unusual drug-induced INR change from 2011 to 2015 in Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology. Results INR was increased unusually in 37 cases(90.2%) and was decreased unusually in 4 cases(9.8%).Intravenous drugs(80.5%) were prone to cause unusual INR change.Prostaglandin,antifungal drugs,antiarrhythmic drugs and lipid soluble vitamins were the commonly used drugs that induced adverse reaction,accounting for 26.8%,29.3%,29.3% and 9.8%,respectively. Conclusion Unusual drug-induced INR change is not rare in elderly patients with warfarin treatment during hospitalization.When other drugs are prescribed,INR should be measured more frequently and the dose of warfarin should be adjusted promptly.

    Objective To share the content and the entry point of the clinical pharmacist participating in the oncology emergency drug treatment. Methods Five typical cases of oncologic emergency occurred in patients with advanced cancer.Clinical pharmacists participated in the drug treatment of circulatory system of emergency,nervous system emergency,metabolic system emergency,blood system emergency and multi-drug resistant bacteria infection.Clinical pharmacists provided suggestions for clinical medication and a whole-course pharmaceutical care by using pharmaceutical knowledge and reviewing relevant information material. Results The work of pharmacists get clinical recognition by assisting clinicians to solve the critical emergencies of cancer patients and improving the effect of drug treatment. Conclusion Clinical pharmacists can be gradually integrated into the medical team and play a critical role as clinical pharmacists in terms of drug treatment.

    Objective To survey the correlation among ADR performances of Shuxuening injection(SXN injection ) based on ADR reports and literature data by rough set theory(RST). Methods By collecting 468 ADR reports of SXN injection,data attribute reduction were processed by Rossata software,and the generated rules and each indicator were analyzed. Results 9 main indicators,which mainly belongs to medication information,had a strong relationship with ADR.There was a correlation among ADR performances.Relationship could also found between performances and medication information. Conclusion The results of the study provide reference for clinical reasonable application of SXN injection,and enrich the research method of ADR.
